Diplomates first certified in 2005 and 2015 should  use this form to submit recertification documents for committee review.  Submissions are due by 11:59 pm ET on June 30, 2025. 

Please ensure that your address is up to date upon submission and kept up to date.  Certificates will be mailed in August.  If certificates are returned as undeliverable to the Diplomate address on file, a $10 re-mailing fee will be charged in addition to applicable postage.
