As Diplomates retire from the practice of dental public health, they may apply for Life Membership. Please review the information below carefully and then use this link to complete your application form. Criteria for Life MembershipA Diplomate may apply for Life Membership if they meet criteria from one of the following four categories. The Board will review applications for Life Membership. The Diplomate will be notified via e-mail of the Board’s decision. Applications may be submitted at any time; however, certificates will be issued in December of each year.No Fee ApplicationsCategory 1. Active Diplomates for 20 years or more, who are completely retired from the practice of Dentistry, Dental Public Health, or Public Health.Category 2. Active Diplomates for 10 years or more, who are age 65 years or older and completely retired from the practice of Dentistry, Dental Public Health, or Public Health.One-Time Fee ApplicationsFees are listed in the Fee Schedule within the Diplomate's Guide--Applicants will be invoiced following application.Category 3. Active Diplomates for fewer than 10 years, who are completely retired from the practice of Dentistry, Dental Public Health, or Public Health.Category 4. Active Diplomates retired from the full‐time practice of Dentistry, Dental Public Health, or Public Health; currently working in Dentistry, Dental Public Health, or Public Health part-time (<200 hours per year / 0.1 FTE). Applications will be reviewed by the Board at the next regularly scheduled Board meeting and applicants will be notified of the Board’s decision thereafter. |
